If we are not in network with your plan, and you need to use out of network benefits, we can provide what is known as a Superbill. A Superbill is a form that is used by members who need to be reimbursed for services by their insurance company. Typically you might know of this form because of a pharmacy expense of some kind. It is also used in mental health as members can often be reimbursed by their insurance plan in whole or partially for the out of pocket cost for therapy.

This form can be quite technical for the average person, but it is very specific and has all of the information an insurance company needs to process a claim. The type of information that you will find on a superbly includes, date of service, provide identification numbers such as an NPI and a Tax ID, therapy service codes, diagnosis code, and more. Because of this, it is important to safeguard your superbills if you do request them.
